Healthcare Reimbursement Account (HRA) Claim Forms

Healthcare reimbursement account (HRA) forms are available at the Trust’s website at: (www.ourbenefitoffice.com/DPFRHC/Benefits). All requests for reimbursement must be completed on this form and sent to BeneSys for review, confirmation and payment.

Please refrain from sending any further HRA Claim Forms to our prior Third Party Administrator (ABS) as it will only delay any claim that you submit. Claims submitted prior to December 21, 2018 may still be processed by ABS.
